Hierbei steht die von Kernberg so benannte expressive Psychotherapie im Mittelpunkt, die er unter Einbeziehung von Behandlungsfällen darstellt und gleichzeitig gegen die psychoanalytische Standardmethode und die stützende Psychotherapie abgrenzt. Bei bestimmten Patientengruppen, wie etwa Suizidgefährdeten oder Antisozialen, sind besondere Behandlungsstrategien notwendig, die ein geändertes therapeutisches Setting erfordern. Die Behandlung von schweren Persönlichkeitsstörungen macht in vielen Fällen einen stationären Aufenthalt notwendig.Der letzte Buchabschnitt ist daher der klinischen Behandlung gewidmet. Kernberg entwirft das Modell einer therapeutischen Gemeinschaft, in das alle für den Behandlungsprozeß wichtigen Elemente integriert sind. Die psychotherapeutische Behandlung schwerer Persönlichkeitsstörungen birgt Risiken und Gefahren, die sich aus dem geänderten Umgang mit der Übertragung, der Interpretation und der technischen Neutralität ergeben.
